Lupe Hammond, a staff member at Hopsice of Lenawee for the past 5 years spoke to our club about a document everyone should have - The Five Wishes.
The Five Wishes DOCUMENT is all you need to start thinking about and documenting your wishes. You will receive 5 copies of the Five Wishes advance directive - one for yourself, and four more to share with those closest to you. The kit also includes the Five Wishes Conversation Guide for Individuals & Families to help you talk about Five Wishes with your doctor and loved ones.
Five Wishes is a comprehensive, easy-to-use advance directive that guides end-of-life care planning, covering personal, emotional, spiritual and medical needs. It is legally recognized in 42 states (including California) as a valid document for appointing a healthcare agent, specifying treatment preferences, and ensuring comfort.
Key Components:
Personal Treatment: Describing how you want to be treated, including dignity and respect.
Final Messages: Conveying what you want your loved ones to know
Healthcare Agent: Choosing who will make care decisions for you.
Comfort Care: Outlining your desires for comfort and pain management.
Thank you, Lupe for suh an important presentation!
